The Health Service of Castilla-La Mancha (SESCAM) is developing this European Health Prevention and Promotion Initiative, in which Nursing professionals actively participatebeing nurses who lead community intervention.

This protocol has integrated a total of 3,470 people over 70 years between June 2024 and September 2025, of which near 28% showed high probability of deterioration of their healthsuch as mobility, falls, comorbidities or losses of their capacities to perform day -to -day activities.

This initiative is part of the comprehensive approach plan of chronicity and the prevention of the fragility of Castilla-La Mancha, which is one of the nine specific plans of the health plan in the region.
